Vida Wellness IV Membership Agreement & Cancellation Policy
By enrolling in a Vida Wellness IV Membership (“Membership”), you (“Member”) agree to the following terms and conditions:
1. Membership Billing & Minimum Commitment
This Membership constitutes a recurring monthly service. The Member authorizes Vida Wellness to automatically charge the payment method on file each month at the agreed-upon rate.
All Memberships require a minimum commitment of three (3) consecutive months from the initial enrollment date. Early cancellation prior to fulfillment of the minimum term is not permitted.
2. Cancellation Policy
All cancellation requests must be submitted in writing via email to info@myvidawellness.com. Requests submitted through any other method will not be accepted.
Cancellation requests will become effective at the end of the current billing cycle, provided the three (3) month minimum commitment has been satisfied. Vida Wellness does not provide prorated refunds for partial billing periods.
3. Refund Policy
All membership fees are non-refundable once charged. This includes, but is not limited to, unused services, missed appointments, or failure to utilize Membership benefits.
No refunds or credits will be issued for any unused IV services unless otherwise required by applicable law.
4. Rollover Policy
Monthly IV services included in the Membership must be used within three (3) months of subscription. Services not used within this timeframe will expire and will not be carried forward.
Exceptions to this policy may be granted solely at the discretion of Vida Wellness and must be requested in writing prior to expiration.
5. Membership Suspension (Pause Policy)
Requests to suspend or pause a Membership must be submitted in writing via email to [info@myvidawellness.com].
All suspension requests are subject to approval by Vida Wellness. Approved suspensions may be limited in duration and may not extend the initial three (3) month minimum commitment unless expressly agreed to in writing.
6. Late Payments & Failed Charges
If a scheduled payment is declined or fails for any reason, Vida Wellness reserves the right to:
* Retry the payment method on file
* Suspend Membership benefits until payment is successfully processed
* Charge a late fee of up to $25 (or the maximum allowed by law).
Continued failure to remit payment may result in termination of the Membership and collection efforts where permitted by law.
7. Reactivation Policy
If a Membership is canceled or terminated, the Member may request reactivation subject to current pricing, terms, and availability. Vida Wellness reserves the right to:
* Require payment of any outstanding balances prior to reactivation
* Apply a reactivation fee
* Enroll the Member under updated Membership terms and pricing
8. Modification of Terms
Vida Wellness reserves the right to modify these terms and conditions at any time. Members will be notified of any material changes, and continued use of the Membership constitutes acceptance of such changes.
9. Non-Transferability
Membership benefits are non-transferable and may only be used by the Member listed on the account.
